Note for Early Childhood (Infant - 4 year olds)

If your child is in school, please enter their current grade level. If your child is an Infant - 4 year old not in PreK, please leave the grade field blank.

Please add any additional household members. For kids, select the grade level for the 2023-2024 school year. Leave grade level blank if your child is not school age.
